Key Responsibilities:
Complete comprehensive nursing assessments on all patients within 8 hours of admission Develop individualized care plans in collaboration with patients, families, and the interdisciplinary team Participate actively in team conferences, providing patient updates and assessments Maintain accurate and timely documentation of assessments, interventions, and patient responses Educate patients and families to support rehabilitation goals and discharge planning from day one Foster patient and team safety, demonstrating strong awareness in safe patient handling, infection control, and environmental safety Communicate significant patient status changes to the care team to maintain continuity of care Supervise and delegate tasks to ancillary personnel, providing guidance and support Promote a positive team culture and participate in unit-based team-building activities Proactively identify personal professional development opportunities
Desired Qualifications:
Active RN license Experience in rehabilitation and/or long-term care nursing Strong communication, critical thinking, and documentation skills Commitment to patient and workplace safety protocols
