  • PRN Summary We are seeking a dedicated PRN Registered Nurse to join our compassionate team at Community Hospice Care.
In this role, you will be instrumental in coordinating patient care, ensuring that our clients receive the highest level of support during their end-of-life journey. Your expertise will help enhance the quality of life for both patients and their families. Responsibilities Coordinate and manage patient care plans to ensure comprehensive support for clients. Conduct thorough assessments of patients' medical needs and develop individualized care plans. Monitor vital signs and manage medications as per physician orders. Provide emotional support and education to patients and families regarding end-of-life options. Collaborate with interdisciplinary teams to facilitate optimal patient outcomes. Maintain accurate medical records in compliance with HIPAA regulations. Advocate for patients' needs and preferences throughout their care journey. Requirements Current RN license in the state of Michigan. Experience in hospice or palliative care preferred but not required. Strong understanding of HIPAA regulations and patient confidentiality. Excellent communication skills for effective interaction with patients, families, and healthcare teams. Ability to work independently while managing multiple cases efficiently. Experience with medical records management is a plus.
