The Care Team Hospice is seeking a full-time Registered Nurse Case Manager to provide skilled nursing care and coordinated end-of-life support to patients in their homes and facility settings while collaborating closely with the interdisciplinary team. Key Responsibilities Conduct comprehensive patient assessments and develop individualized plans of care Provide skilled nursing care in accordance with hospice standards and physician orders Coordinate care with physicians, caregivers, and interdisciplinary team members Educate patients and families on symptom management, disease progression, and end-of-life care Document assessments, interventions, and care plans accurately and in a timely manner in the
EMR HCHB
(Homecare Homebase) Participate in interdisciplinary team meetings and care planning Support after-hours patient needs as part of an on-call rotation when applicable Qualifications Active Registered Nurse (RN) license Hospice or home health experience preferred Strong clinical assessment and care coordination skills Ability to work independently in the field Strong communication and documentation skills Experience using HCHB (Homecare Homebase) preferred Field Role Requirements Active driver's license Ability to pass a criminal background check Ability to pass a Motor Vehicle Record (MVR) check Current TB testing in accordance with agency policy Current CPR certification as required for the role
