Nursing Full Job Description Position Overview:
We are seeking a skilled and compassionate Registered Nurse (RN) to join our team in a PRN (as needed) capacity in the home health care setting. As a Registered Nurse in home health care, you will play a crucial role in providing high-quality and personalized care to our homebound patients. Your primary responsibility will be to assess patients, develop individualized care plans, and implement comprehensive nursing interventions to promote their overall well-being and independence.
Responsibilities:
Patient Assessment and Care Planning:
Conduct thorough and comprehensive assessments of patients' physical, emotional, and psychological needs in their home environment. Develop individualized care plans based on the assessment findings, considering the patient's health conditions, goals, and support systems.
Skilled Nursing Interventions:
Administer and manage medications, wound care, and various medical treatments as prescribed by the physician. Provide skilled nursing care, including catheter care, IV therapy, and other specialized procedures as required.
Patient Education and Support:
Educate patients and their families on disease management, medication adherence, and self-care techniques to promote optimal health and independence. Offer emotional support and counseling to patients and their families to address any concerns and facilitate coping with health challenges.
Documentation and Reporting:
Maintain accurate and detailed patient records, documenting assessments, care provided, and patient responses to interventions. Collaborate with the healthcare team, including physicians and therapists, to ensure effective communication and continuity of care.
Safety and Infection Control:
Adhere to strict safety protocols and infection control measures to promote a safe and sanitary environment for patient care. Evaluate the home environment for potential hazards and provide recommendations for modifications to enhance patient safety.
Care Coordination:
Coordinate with other healthcare professionals, such as physical therapists, occupational therapists, and social workers, to optimize patient outcomes. Participate in case conferences and interdisciplinary team meetings to discuss patient progress and adjust care plans as needed.
Qualifications:
Education and Licensure:
Graduation from an accredited nursing program with a current and valid RN license in the state of practice.
Experience:
Prior experience in home health care or a related field is preferred but not required. Familiarity with providing skilled nursing care, wound care, and medication management.
Interpersonal Skills:
Excellent communication and interpersonal skills to interact effectively with patients, families, and the healthcare team. Compassionate, patient-centered approach with a commitment to delivering high-quality care.
Organizational Skills:
Strong organizational skills to manage patient schedules, documentation, and care plans effectively.
Flexibility:
Ability to work in a PRN capacity, adjusting availability based on patient caseload and organizational needs.
