Monday-Friday 8:00 AM-4:30 PM/ on call rotating hours
Pay:
$38.00-$47.00 The Triage Nurse Under the supervision of and in collaboration with the Clinic Manager, provides oversight and management of the overall clinical and operational effectiveness of the PACE clinic to ensure consistency and high-quality care is provided to program participants.
Required Qualifications:
To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. The requirements listed below are representative of the knowledge, skill and/or ability required. Associated Degree in Nursing (RN) Unencumbered license as a Registered Nurse (RN) in the State employed required A minimum of one year's experience in working with the frail or elderly population. Must have a valid driver's license, proof of insurance and have means of transportation. Must have medical clearance for communicable diseases and up-to-date immunizations before having direct participant contact.
Essentials:
In collaboration with the Clinic Manager provides oversight, management and direction of the overall operational and clinical effectiveness of the PACE clinic. In collaboration with program leadership, provides oversight, management, and coordination of the consistency and quality of the twenty-four (24) hour care provided to program participants. Provides direct supervision and support to the nurses. Assess the acute needs of participants either via telephone or in-person triage, prioritizing participant needs based on urgency and acuity. Provide clinical advice, recommendations for symptom management and recommendations for appropriate medical interventions that are within the nursing scope of practice, referring to providers when necessary. Acts as the interim Clinic Manager in his/her absence for vacation, illness, etc. In collaboration with the Clinic Manager, oversees the provision of nursing care ensures all ordered, approved, or care planned nursing care and services are delivered timely and in accordance with nursing standards or practice.
