Qualifications RN License Full Job Description Overview:
The Home Health RN delivers compassionate, patient-centered nursing care in a home environment. This role focuses on helping patients recover, manage chronic conditions, and maintain independence while ensuring high-quality clinical outcomes.
Primary Duties:
Conduct comprehensive in-home health assessments and ongoing evaluations Create and manage personalized care plans in collaboration with physicians Provide skilled nursing services including injections, IV therapy, catheter care, and wound management Track patient conditions and adjust care plans as needed Maintain detailed and timely clinical documentation Teach patients and caregivers about treatments, medications, and preventive care Communicate regularly with healthcare providers and interdisciplinary team members Follow infection control practices and ensure patient safety at all times
Required Qualifications:
Current RN license in good standing Degree from an accredited nursing program Valid CPR/BLS certification At least 1 year of nursing experience (home health or community care preferred) Excellent clinical judgment and problem-solving abilities Strong interpersonal and communication skills Ability to travel to patient homes and work independently
Work Setting:
Care is provided in private residences or assisted living settings Frequent local travel required Flexible work hours based on patient needs
