Weekday visits, weekend availability, on-call rotation, flexible home visit schedules, and patient-based assignments may be available. Position Summary KARE Masters Home Health is seeking Registered Nurses to provide skilled home health care services to patients in their homes. This role supports patients recovering from illness, injury, surgery, disability, or chronic health conditions. The Home Health RN provides skilled observation and assessment, care coordination, patient education, medication teaching, wound care support, chronic disease management, and communication with physicians, families, and the care team. Key Responsibilities Complete skilled nursing assessments and patient evaluations. Monitor patient condition and report changes to appropriate providers. Provide wound care, post-surgical care, and skilled observation. Support chronic disease management for conditions such as diabetes, hypertension, CHF, and COPD. Provide medication teaching and patient/caregiver education. Assist with care planning and coordination with physicians, families, and care team members. Document services accurately and timely according to agency standards. Support fall prevention, safety, and rehabilitation goals. Provide catheter care, ostomy care, pain management support, and other skilled nursing services as assigned. Maintain compliance with company policies, clinical standards, confidentiality rules, and safety guidelines. Qualifications Active Michigan Registered Nurse license required. Home health, skilled nursing, hospital, rehabilitation, or long-term care experience preferred. Strong assessment, communication, and documentation skills required. Ability to work independently in client homes. Reliable transportation required. Must be comfortable traveling to patient homes throughout assigned service areas. Must be able to complete required onboarding, background checks, and health screenings. CPR certification preferred or required based on agency policy.
