Home Health Territories include Jackson, Grass Lake, Manchester and other surrounding areas. What You Will Do Provide care across multiple patient homes within designated service areas or regions Incorporate and demonstrate standard elements of professional practice: Assess, Outcomes Identification/Solutions, Plan, Implement, Evaluate Act independently within license, scope of knowledge, and experience Retain accountability for delegation, decisions, and outcomes Collaborate with key stakeholders and contribute to quality improvement Exhibit agility and willingness to take on new responsibilities Embrace new ideas and cultural differences while managing competing priorities Minimum Qualifications Current Registered Nurse license in the state of practice (no flags or restrictions) Current BLS Certification from the American Heart Association Minimum of two years of acute care nursing experience Current Driver's License, reliable transportation, and automobile insurance Home care experience preferred Preferred Clinical Skills& Experience Foley and Supra Pubic Catheters

  • sterile technique and site monitoring G-Tube Management
  • feeding, flushing, site care Medical Management
