Develop, update, and follow individualized care plans in collaboration with physicians, social workers, and the broader care team. Educate and support family caregivers on disease progression, what to expect, and medication management. Respond promptly to changes in patient condition, helping manage crises and maintain comfort. Complete accurate, up-to-date documentation and support effective communication across the care team. Provide compassionate emotional and spiritual support, including grief counseling and post-mortem support to grieving relatives. What You Need Required Current RN license Ability to provide compassionate end-of-life nursing care, including pain and symptom management Preferred Hospice experience and comfort working with patients and families through serious illness and bereavement Strong clinical assessment skills and documentation accuracy About the Assignment This hospice RN opportunity supports patients focused on comfort and quality of life, coordinating care with a multidisciplinary team and offering education and emotional support to families throughout the end-of-life journey. You'll help ensure symptoms are managed effectively, care plans are kept current, and communication remains clear across the team.
