Job Description
Hospice Community Registered Nurse (RN) - Contingent Days Blue Water Home Care & Hospice Join a team that has proudly served our community for over 70 years . Blue Water Home Care & Hospice is a Port Huron-based, non-profit organization providing high-quality, cost-effective home health and hospice care to patients and families throughout St. Clair, southern Sanilac, and northern Macomb counties . Position Summary The Hospice Registered Nurse provides skilled, compassionate end-of-life nursing care to patients and families. This role focuses on palliative care, symptom management, and comfort while supporting patients' physical, emotional, and psychosocial needs. The RN collaborates closely with physicians and the interdisciplinary team to ensure care aligns with the individualized plan of care and patient goals. Essential Responsibilities Perform comprehensive initial and ongoing nursing assessments using the nursing process Develop, implement, and update individualized plans of care based on patient needs and physician orders Provide skilled hospice nursing care with a focus on symptom control, comfort, and quality of life Educate patients and families on disease progression, end-of-life care, and ethical considerations Assess caregiver ability and provide ongoing education and support Coordinate and collaborate with physicians, hospitals, skilled nursing facilities, and interdisciplinary team members Identify and initiate appropriate interdisciplinary referrals and document care accurately Monitor patient status and communicate changes to the physician and care team Supervise LPNs, hospice aides, and paraprofessional staff in compliance with regulatory requirements Complete supervisory visits for hospice aides at least every 14 days, including annual onsite supervision Determine hospice eligibility and apply admission and recertification criteria Prepare accurate clinical documentation, progress notes, and chart reviews Participate in interdisciplinary team (IDT) meetings as assigned Utilize electronic systems including RXNT and Enclara Participate in quality improvement initiatives and the on-call rotation as required Additional Responsibilities Attend required meetings, trainings, and in-service programs Adhere to agency policies, procedures, and regulatory standards Maintain confidentiality of patient, employee, and agency information Foster positive working relationships through effective communication Embrace and promote a culture of kindness with a patient- and family-centered approach Perform other duties as assigned Qualifications Education & Experience Graduate of an accredited school of professional nursing Hospice or palliative care experience preferred Licensure & Certifications Active, unencumbered Registered Nurse (RN) license in the State of Michigan Hospice and/or palliative care certification preferred Current CPR/BLS certification Valid driver's license, automobile insurance, and reliable transportation Skills & Abilities Strong clinical judgment and critical thinking skills Ability to work independently and as part of an interdisciplinary team Excellent communication and documentation skills Ability to manage stressful situations with professionalism and compassion Work Environment Care provided primarily in patient homes Routine exposure to blood and bodily fluids Occasional exposure to environmental hazards, inclement weather, and traffic conditions Ability to manage emotionally demanding situations with calm and professionalism Physical Requirements Frequent travel by automobile Ability to enter and exit various home environments independently Ability to lift up to 50 lbs occasionally Frequent standing, walking, reaching, grasping, and fine motor skills Why Blue Water Home Care & Hospice?
