Hospice Case Manager

Hospice RN Case Manager Location:

Monroe, Michigan, 48162 This is a full-time hospice Registered Nurse (RN) case management role based in Michigan. The nurse provides direct patient care to hospice patients in home or facility settings while coordinating care with an interdisciplinary team. Key Responsibilities Perform patient assessments and ongoing hospice care Develop and manage individualized care plans Coordinate with physicians, LPNs, CNAs, and support staff Educate patients and families on hospice and end-of-life care Schedule & Workload Monday-Friday, 8 AM-5

PM On-call:

1 weeknight and 1 weekend shift every other week Approximately 6 patient visits per day Travel within a 40-mile radius 40 guaranteed hours per week 13-week contract Requirements Active RN license BLS certification Valid driver's license and auto insurance Minimum 1 year RN experience (hospital, hospice, home health, oncology, or long-term care) Strong communication, assessment, and documentation skills Reliable transportation required Preferred Experience with HCHB (Homecare Homebase) documentation system Hospice or home health experience preferred but not mandatory Don't miss this opportunity•Be a part of one the best health care facility !!
