Lakeshore Home Health Care Services is seeking a dedicated RN Care Coordinator to join our Muskegon office , working closely with our experienced Wyoming team—two amazing nurses who've been part of the Lakeshore family for over a decade.

What You'll Do:
Conduct in-home supervisory visits for our adult clients and home health aides. Perform comprehensive assessments and update care plans in collaboration with clients and families. Manage med setups , physician orders , and care plan updates . Assist with case openings , aide training , and respond to client needs and changes.
