QUALIFICATIONS
Current Michigan License as Registered Nurse. BSN preferred with experience in community based care Possesses and maintains a current CPR certification. Current driver's license.
SUMMARY OF JOB RESPONSIBILITIES
Responsible for patient referrals and intake activity; liaison with area discharge planners, referrers, payers and patients/families. Reasonable accommodations may be made to enable individuals with disabilities to perform the essential functions
ESSENTIAL FUNCTIONS AND DUTIES
(other duties may be assigned) Facilitates hospice referrals by physicians and hospital discharge planners by liaison activity at area hospitals as needed Assesses patient/family interest in hospice care and services as needed Provides information on hospice care and programs to referral sources and patients and families Assesses and documents appropriateness for hospice at initial assessment visit Consults with Hospice Physician to determine appropriateness for hospice and obtain approval for admission Conducts initial nursing assessment visit and arranges for services to begin Consults with Hospice Physician and/or clinical manager on anticipate needs for new admissions and arranges for those needs as appropriate Communicates with attending physician to inform them of referral status Determines payer source and coordinates the determination of hospice benefits for the referred patient as needed Coordinates with discharge planners and Homecare/Poppen staff to assure continuity of care and patient admission to the hospice program as needed Completes written admission documentation with patients and families as needed Is knowledgeable of the ICD coding system and is able to identify appropriate codes at time of admission Assist with Homecare or Poppen patient visits as needed Carries out projects and tasks related to community and public relations as assigned
