Job Description

Registered Nurse Supports Coordinator :
Responsible for in-home assessments, care plans, community referrals, medication review, and ongoing case monitoring for an elderly and disabled adult population. Candidate must have current Michigan Registered Nurse license, the ability to work from a home office, have high-speed internet access at home (10MB+), and functional computer skills. Position requires transportation for travel to participant homes in Mecosta, Osceola, & adjacent counties . Position is eligible for mileage reimbursement. Experience in Geriatrics, Home Health Care or Case Management preferred. Bi-Lingual Spanish speaking candidate a plus but not required . This is a Full time 40-hour position, with no holidays or weekends required! The starting salary range is $62,000 - $64,500 plus mileage reimbursement, an excellent benefits package and flexible work schedule.
