This is a 12-month contract opportunity.

One available Monday, Tuesday, and Wednesday One available Wednesday, Thursday, and Friday One available Rotating schedule: Week 1: Saturday & Sunday + Week 2: Monday & Tuesday

ADDITIONAL REQUIREMENTS
Current unrestricted RN license in Michigan. Minimum 2 years of clinical nursing experience, with a background in case management preferred. Certification in Case Management (CCM) preferred. Proficiency in using telehealth platforms and electronic health records (EHR) systems. Excellent communication skills, both verbal and written. Strong critical thinking and problem-solving abilities. Ability to work independently and as part of a multidisciplinary team. Compassionate and empathetic approach to patient care. Flexibility to adapt to changing healthcare environments and patient needs. Commitment to maintaining patient confidentiality and privacy. Bilingual Russian/Spanish/Arabic - Preferred
JOB RESPONSIBILITIES
Conduct initial patient assessments and develop individualized care plans based on clinical findings and patient needs. Comfortable managing a high-volume caseload of ~200 Medicare members while meeting quality and performance standards Utilize telehealth platforms to monitor patient progress, provide health education, and coordinate necessary interventions. Collaborate with healthcare providers, including physicians, specialists, and allied health professionals, to ensure comprehensive care delivery. Assess and address patient concerns, escalating issues as needed to ensure timely resolution. Educate patients and caregivers on health management strategies, medications, and self-care techniques. Document all patient interactions, assessments, and care plans accurately and timely within electronic health records (EHR) systems. Coordinate referrals to community resources and support services as necessary. Stay informed about current healthcare trends, telehealth regulations, and best practices in case management. Comfortable on production, meeting production case load targets and exceeding quality expectations
