Job Description

This role requires an onsite presence at both Royal Oak and Dearborn Hospitals Job Summary This position will work closely with the Imaging Directors and service line leaders to drive strategic initiatives within MRI. These goals will focus on aligning patients being seen within the right care setting, helping drive efficient access for critical patients within hospital settings. The nurse navigator will also work closely with the Anesthesia team and guidelines to prioritize patients using the lowest risk methods for greatest benefit to achieve quality outcomes. Looking for an individual with a strong ability to drive change, passionate about patient care, and strong communication and organizational skills. Essential Functions Assesses the educational needs of clients/others. Develops and implements appropriate educational curricula/teaching strategies or makes appropriate referrals. Provides ongoing review and development of educational materials related to clinical management. Case manages the population both in the inpatient and/or outpatient settings. Develops and manages clinical paths as appropriate, and measures outcomes of care to maximize continuity of care and appropriate resource utilization and length of stay. Coordinates multispecialty team clinics. Provides direction to specified projects/programs pertaining to the population, assessing, planning, developing/maintaining, implementing and evaluating outcomes of same. Documents educational process/patient information according to program/MDPH guidelines. Communicates, collaborates, networks with and acts as a consultant to clients/significant others, other members of the health care team, and the community in order to ensure continuity of care and coordination of services, and to provide needed resources to the community. Writes and reviews protocols, procedures and guidelines pertaining to clinical management.
