This role involves delivering personalized care in patients' homes, ensuring their comfort, safety, and well-being. The ideal candidate will be dedicated to promoting health, managing chronic conditions, and supporting patients and their families through effective communication and clinical expertise. Responsibilities Provide direct nursing care and assessments in patients' homes in Taylor and nearby communities. Develop and implement individualized care plans based on patient needs and physician orders. Monitor patient progress and report any changes in condition to the healthcare team. Educate patients and families on disease management, medication administration, and health maintenance. Ensure compliance with all regulatory requirements and company policies. Requirements Current Registered Nurse (RN) license in the state of Michigan. Previous home health care experience preferred but not required. Ability to work independently and manage multiple patient visits efficiently. Excellent communication and interpersonal skills.
