As a Hospice RN Case Manager , you will play a pivotal role in providing personalized, patient-centered care to individuals in their end-of-life journey, ensuring comfort, dignity, and peace. You will work closely with patients, families, and interdisciplinary teams to create and implement comprehensive care plans tailored to meet the unique needs of each individual. Your expertise and empathy will be vital in managing symptoms, providing emotional support, and facilitating communication among all involved. Responsibilities Assess patients' health care needs and develop individualized care plans. Coordinate patient care with physicians, family members, and other healthcare professionals. Ensure compliance with state and federal regulatory requirements in hospice care. Monitor patients' symptoms, medication responses, and overall progress. Provide education and emotional support to patients and their families. Facilitate and attend interdisciplinary team meetings to discuss patient care. Document patient care accurately and in a timely manner in accordance with company policies.
