Responsibilities:
Functions as a liaison for management staff, administration, physicians, managed care companies, community organizations, and other customers. Coordinates efforts for treatment and promotes quality improvement and educational efforts. Compiles data, tracks outcomes, and makes recommendations for process improvement. Provides referrals/support for follow-up care for patients who are uninsured or underinsured.Location:

8AM-4:30PM M-F (no weekends, no on-call, no Holidays, no OT)Benefits:
Full-TimeEDUCATION/EXPERIENCE REQUIRED
Bachelor's degree in nursing (BSN) required Minimum of 3 years' experience as a Registered Nurse (Med-surge or Ambulatory preferred) Experience with patient care plans preferred. Comfortable with Microsoft office suite applications (excel, word, calendar, outlook and teams meetings) Comfortable interacting with patients both face-to-face and telephonic Comfortable communicating with other disciplines (physicians, social workers, office staff, community health) Comfortable documenting in EHR as well as navigating EHRs Strong communication skills, organizational planning and implementation skills and the ability to handle multiple complex patients' needs simultaneouslyCERTIFICATION/LICENCES REQUIRED
Registered Nurse credentialed from the Michigan Board of Nursing obtained prior to hire date or job transfer date required.
