Warren, MI Job Details Part-time $500 - $2,000 a month 7 hours ago Qualifications RN License Hospice experience Hospice care BLS Certification Driver's License Nursing Full Job Description Hospice RN On-Call Triage Nurse (Part-Time/Per Diem) Proactive Hospice Care LLC Proactive Hospice Care is seeking a compassionate, experienced, and dedicated Hospice Registered Nurse (RN) to join our on-call team. This position is ideal for an RN who is passionate about providing exceptional end-of-life care while supporting patients and families after hours. We are currently seeking coverage for Thursday, Friday, Saturday, and Sunday , including after-hours triage calls and field visits as needed. Position Responsibilities Respond to after-hours calls from hospice patients, families, caregivers, and facilities. Perform clinical triage over the phone and provide symptom management recommendations. Determine when an in-person nursing visit is required. Open new hospice admissions (Start of Care) during on-call coverage when needed. Complete routine and PRN hospice visits during the assigned on-call days. Conduct emergency nursing visits as clinically indicated. Perform death pronouncements and post-mortem visits when required. Coordinate care with the Medical Director, interdisciplinary team, pharmacies, and DME providers. Accurately document all patient interactions, visits, and interventions in the hospice EMR. Communicate significant changes in patient condition to the interdisciplinary team and on-call physician. Provide education, emotional support, and compassionate guidance to patients and their families.

Schedule On-Call Coverage:

Thursday, Friday, Saturday, and Sunday Flexible monthly schedule (few days each month) Phone triage from home with field visits as needed Compensation Competitive monthly on-call stipend. All patient visits, admissions (SOC), PRN visits, and death visits are paid separately in addition to the monthly on-call rate. Phone forwarding service provided during assigned on-call coverage. Qualifications Current Registered Nurse (RN) license in the State of Michigan. Minimum of 2 years of RN experience required. Previous Hospice or Palliative Care experience strongly preferred. Strong clinical assessment, triage, and critical thinking skills. Ability to work independently and make sound clinical decisions. Excellent communication and documentation skills.
