The Registered Nurse Case Manager supports hospice patients receiving care in their homes or facility settings by providing skilled nursing care and coordinated, compassionate end-of-life support in partnership with the interdisciplinary team. EMR HCHB
(Homecare Homebase) Participate in interdisciplinary team meetings and care planning Support after-hours patient needs as part of an on-call rotation when applicable Active Registered Nurse (RN) license Hospice or home health experience preferred Strong clinical assessment and care coordination skills Ability to work independently in the field Strong communication and documentation skills Experience using HCHB (Homecare Homebase) preferred Field Role Requirements Active driver's license Ability to pass a criminal background check Ability to pass a Motor Vehicle Record (MVR) check Current TB testing in accordance with agency policy Current CPR certification as required for the role
