Job Description

Hospice Registered Nurse Part-time hours, flexibility, and meaningful hospice work that allows you to focus on quality patient care. Looking for meaningful nursing work with flexibility? The Care Team Hospice is seeking a compassionate Part-Time Registered Nurse to provide exceptional end-of-life care to patients in facility settings and private homes. Join a supportive team environment where you can make a meaningful impact while delivering compassionate, patient-centered hospice care to patients and families during life's most important moments. Schedule, Hours & Territory Standard operating hours are Monday through Friday, 8:00 AM to 4:30 PM Territory includes West Branch, MI, and surrounding areas within approximately a one-hour radius based on patient census and branch needs. What We Offer Strong interdisciplinary team collaboration Supportive clinical leadership and resources Clear expectations and consistent workflows Environment focused on quality, compliance, and patient-centered care Key Responsibilities Conduct comprehensive patient assessments and develop individualized plans of care Provide skilled nursing care in accordance with hospice standards and physician orders Coordinate care with physicians, caregivers, and interdisciplinary team members Educate patients and families on symptom management, disease progression, and end-of-life care Document assessments, interventions, and care plans accurately and on time in the
EMR HCHB
(Homecare Homebase) Participate in interdisciplinary team meetings and care planning Support after-hours patient needs as part of an on-call rotation when applicable Active Registered Nurse (RN) license Hospice or home health experience preferred Strong clinical assessment and care coordination skills Ability to work independently in the field Strong communication and documentation skills Experience using HCHB (Homecare Homebase) preferred Field Role Requirements Active driver's license Ability to pass a criminal background check Ability to pass a Motor Vehicle Record (MVR) check Current TB testing in accordance with agency policy Current CPR certification as required for the role
