Schedule/Hours Travel assignment, schedule details provided upon interview. Qualifications Active RN licensure in the state or compact eligible. Minimum of one year of experience in Rehab or Long-Term Care nursing preferred. BLS (Basic Life Support) certification required. Strong assessment, communication, and documentation skills. Demonstrated commitment to safe patient handling and safety protocols. Responsibilities Conduct comprehensive assessments on all patients within 8 hours of admission and formulate individualized plans of care. Collaborate with patients, families, and the interdisciplinary team to identify needs and establish teaching goals. Participate in team conferences and ensure timely, accurate charting of all care-related activities and observations. Deliver direct patient care using the nursing process, with an emphasis on education and rehabilitation outcomes. Initiate discharge planning at admission and provide ongoing patient and family education. Communicate effectively with physicians, charge nurses, and ancillary staff to ensure continuity and quality of care. Supervise and provide leadership to ancillary nursing personnel; delegate responsibilities appropriately. Maintain environmental and personal safety, while promoting a positive and efficient work atmosphere. Assume charge responsibilities as assigned and proactively pursue professional development.
