What You'll Be Doing As an Evening & Overnight Field Nurse, you'll provide skilled hospice care throughout the Greater Twin Cities Metro Area while helping patients remain comfortable and supported in their homes and communities.

Responsibilities include:

Providing evening and overnight nursing visits. Assessing changing patient conditions. Managing pain and symptoms using comfort-focused interventions. Supporting patients and families through education and guidance. Providing care in private homes, assisted living communities, memory care settings, and skilled nursing facilities. Collaborating with physicians, social workers, chaplains, hospice aides, and fellow clinicians. Completing timely and accurate documentation. Helping patients avoid unnecessary hospitalizations whenever possible.
