Patient & Family Consultations Meet with patients and families to discuss hospice services and eligibility. Conduct hospice consultations and educational visits. Help families understand available care options and next steps. Serve as a trusted resource throughout the decision-making process. Care Transitions & Admissions Support timely hospice admissions and patient onboarding. Coordinate transitions from hospitals, senior communities, and other care settings. Collaborate closely with clinical teams to ensure continuity of care. Help remove barriers to accessing hospice services. Referral Partnerships Build and maintain strong relationships with hospitals, providers, discharge planners, case managers, senior living communities, and referral partners. Serve as a clinical resource within the healthcare community. Educate referral sources about hospice services and patient eligibility. Strengthen existing partnerships while developing new opportunities. Community Presence Represent Superior Hospice throughout the Twin Cities Metro Area. Participate in outreach efforts and relationship-building activities. Help increase awareness of hospice services and patient resources. Support organizational growth through trust and credibility. What We're Looking For. Active Minnesota Registered Nurse (RN) License. Hospice experience preferred. Strong consideration given to nurses from ICU , Oncology , Palliative Care , ER , Care Coordination , Case Management , Discharge Planning , or other high-acuity settings. Exceptional communication and relationship-building abilities. Comfortable educating patients, families, and healthcare professionals. Strong clinical judgment and professionalism. Comfortable working independently in a field-based role. Reliable transportation and ability to travel throughout the Greater Twin Cities Metro Area. The Right Nurse For This Role.
