Identify hospice-appropriate patients through referral relationships and community partnerships. Serve as a clinical resource for providers, discharge planners, and referral sources. Conduct hospice consultations and educational visits with patients and families. Support timely admissions and care coordination efforts. Build and maintain relationships across hospitals, SNFs, and senior communities. Support continuity of care and provide field support as needed.What We're Looking For:
Active Minnesota Registered Nurse license. Hospice experience preferred or strong background in ICU, oncology, palliative care, care transitions, or high-acuity nursing. Strong communication and relationship-building abilities. Comfortable working independently in a fast-moving field role. Reliable transportation and ability to travel throughout the Greater Twin Cities Metro Area.
