What You'll Do:
Admissions:
Complete hospice eligibility assessments, patient and family education, consents, medication reconciliation, and start of care activities. Case Management (as assigned): Manage patient care plans, symptom management, caregiver education, and ongoing coordination needs.
Visit Nurse and Crisis Support:
Provide routine, PRN, urgent, and support visits while responding quickly to changing patient needs.
Collaborative Care:
Work closely with the interdisciplinary team to provide aligned care and support patient and family goals.
Documentation and Compliance:
Maintain accurate EMR documentation and follow Medicare hospice regulations and agency standards.
What You'll Bring:
Active Minnesota Registered Nurse license. Hospice experience preferred or strong backgrounds in ICU, palliative care, oncology, ER, or high-acuity nursing settings. Strong communication and patient/family education skills. Comfortable working independently in a field-based role. Organized, decisive, and adaptable in changing situations.
