Job Description

Job Overview Fairview has an immediate opening for an RN to join our team at the Fairview Woodwinds Clinic! This role may require travel to Edina, Riverside, Maplewood, Blaine, and Wyoming. Clinic hours are Monday through Thursday from 7:00 AM to 6:00 PM and Friday from 7:00 AM to 5:00 PM. You will be scheduled within those hours. Works closely with a multidisciplinary health care team to coordinate the treatment process of the patient and family as they manage their pain; and to enhance program growth and development. Responsibilities Coordinates care for patients and families in collaboration with the multidisciplinary team across the continuum of care. Manages the process for a proactive approach to pain management, which includes self-reliance and self-care. Serves as clinical expert/resource to chronic pain patients and their families, the health care team and the community. Maintains communication with referring health care providers regarding a comprehensive plan of care for patients and family. Coordinates the development, implementation, and evaluation of multidisciplinary education plan. Utilize the pain management philosophy of self-care management when providing education to patient, family, and community. Provides phone consultation and triage to patients, families, other healthcare professionals and the community. Supports in clinic procedures, procedure monitoring, post procedure education and discharge. Required Qualifications
A.A./A.S.
in nursing 1 year experience as an RN MN Registered Nurse (RN) License Preferred Qualifications
B.S./B.A.
