You'll care for residents within assisted living, memory care, skilled nursing, and senior housing communities while collaborating closely with nurses, therapists, caregivers, and facility leadership. Providers who thrive here enjoy independent decision-making, problem-solving, and building trusted relationships with patients over time. What You'll Be Doing Conduct routine provider rounding visits throughout Western Minnesota Perform comprehensive patient assessments and ongoing clinical evaluations Manage chronic conditions and medically complex patients Develop and implement individualized care plans Collaborate with physicians, nurses, therapists, and community leadership Support care transitions and continuity of care initiatives Identify opportunities to reduce avoidable hospitalizations and emergency department utilization Provide patient and family education Build trusted relationships with residents, families, and healthcare teams Participate in specialty clinical programs and advanced treatment initiatives Ideal Backgrounds We welcome Nurse Practitioners with experience in: Primary Care Family Practice Internal Medicine Geriatrics Long-Term Care Skilled Nursing Facilities (SNF) Post-Acute Care Home Health Hospital Medicine Hospice & Palliative Care Qualifications Active Minnesota Nurse Practitioner License in good standing Strong clinical assessment and diagnostic skills Comfortable practicing independently while collaborating with physicians and interdisciplinary teams Passion for serving older adults and medically complex populations Reliable transportation and willingness to travel throughout Western Minnesota Experience or interest in advanced wound care , chronic disease management, and specialty clinical services preferred Wound Care Certification (WCC) or prior wound care experience is highly desirable but not required Build a Practice That Matters The best providers don't simply treat conditions.
