This position is ideal for an RN passionate about making a difference in the community by empower individuals to live independently in their homes and communities.
Responsibilities:
Implement and monitor with the client/responsible party a plan of care based on the service delivery plan Train and orient CFSS support workers Supervise, observe, and evaluate worker competency Provide nursing services as directed by the agency Oversee health-related procedures and safety compliance Maintain accurate and timely clinical records, logs, and documentation Participate in internal audits, quality improvement initiatives, and compliance reviews
Qualifications:
A current, unrestricted RN license in Minnesota is required Minimum of 1-2 years of nursing experience in home care, disability services, or community health settings required Ability to travel within the Twin Cities metro with a valid driver's license and a reliable personal transportation with current automobile insurance coverage Excellent communication, documentation, and organization skills Experienced in dealing with clients of various cultural backgrounds Self-motivated and diligent
