As a Weekend Evening Field Nurse , you'll care for patients throughout the Twin Cities Metro Area during evening and overnight hours. You'll provide symptom management, respond to changing needs, and serve as a trusted resource for patients and families navigating complex situations. Every shift brings variety, independence, and the opportunity to have a genuine impact. What You'll Do Provide skilled hospice nursing care during evening and overnight hours. Deliver symptom management and comfort-focused interventions. Assess patients and respond to changing clinical conditions. Support patients and families through education, guidance, and compassionate care. Provide care in private homes, assisted living communities, memory care settings, and skilled nursing facilities. Collaborate with physicians, nurses, social workers, chaplains, and hospice aides. Maintain accurate and timely clinical documentation. Who This Role Is Built For Active Minnesota Registered Nurse (RN) License Hospice experience preferred but not required. Nurses from
ICU , ER
, Oncology , Med Surg , Palliative Care , Home Health , Hospital , and bedside nursing backgrounds. Nurses interested in transitioning into hospice are encouraged to apply. Strong communication and patient education skills. Confidence working independently in a field-based environment. Reliable transportation and ability to travel throughout the Greater Twin Cities Metro Area.
