What You'll Be Doing Lead Patient Care Perform in-home assessments and develop individualized care plans. Manage ongoing visits and clinical decision-making. Use Your Clinical Expertise Perform OASIS assessments and documentation. Manage chronic conditions and complex medical needs. Identify changes in condition and intervene proactively.
