At Legacy Home Care and Home Health Care, Inc., our Floating Visiting Nurses (RNs) deliver skilled nursing care directly in the home while supporting multiple territories across the Twin Cities. This position offers variety in your day while still maintaining a consistent schedule and strong clinical support. In this full-time field role, you will build one-on-one relationships with patients, manage your own daily route, and step in to support nearby territories when needed. As a Visiting Nurse in home care, you will provide skilled nursing visits, complete patient assessments, and educate patients and families on managing their health safely at home. You will also assist nearby territories when needed, helping ensure patients continue to receive consistent, high-quality care. Floating Visiting Nurse | RN Responsibilities Conduct skilled nursing visits and patient assessments in the home. Provide wound care, medication management, and patient education. Develop and update individualized plans of care. Support patients and caregivers with disease management and safety. Float to nearby territories based on patient needs. Communicate with therapists, physicians, and care team members. Complete accurate documentation in EMR and OASIS when required. Participate in limited on-call rotation. Floating Visiting Nurse | RN Qualifications Active Minnesota Registered Nurse license. Valid driver's license and reliable vehicle. Strong clinical and communication skills. Ability to work independently in a field-based role. Home health experience preferred but not required.
