What You'll Own:

Complete Start of Care (SOC) admissions and OASIS documentation. Perform comprehensive in-home assessments . Develop and initiate personalized care plans . Coordinate services and communicate with providers and caregivers. Ensure safe, smooth transitions into home health services.
