Deliver pain and symptom management through a comfort-focused approach. Assess changing patient conditions and provide appropriate clinical interventions. Support patients and families through education, communication, and guidance. Provide care in private homes, assisted living communities, memory care settings, and skilled nursing facilities. Coordinate closely with physicians, social workers, chaplains, hospice aides, and fellow clinicians. Complete timely documentation supporting continuity and quality outcomes.
