POSITION TITLE
: Public Health Nurse

HOURS:
32 hours per week
JOB PURPOSE
Provides professional services in the delivery of health promotion/disease prevention activities and case management primarily focused in Senior Health.
WAGE RANGE
$34.34 - $48.79 PRIMARY
DUTIES AND RESPONSIBILITIES
Provides Professional Public Health services to individuals and families in the community. Participates in health promotion clinics and related activities as assigned. May conduct disease outbreak follow-up and control as assigned. Participates in public health emergency preparedness planning. Completes assessments for eligibility programs offered through state waivers Provides care coordination for various health plans as contracted for senior clients Ability to provide immunizations to all ages when necessary. JOB REQUIREMENTS
Ability to establish and maintain cooperative working relationships with other staff members, representatives of other agencies, clients, and the general public. Ability to utilize effective teaching methods in relating to clients, families, groups and staff. Computer experience required.
MINIMUM QUALIFICATIONS
Current licensure as a Registered Nurse in the State of Minnesota. Subject to criminal history background check.
ESSENTIAL FUNCTIONS
Ability to do visits in homes that may not be handicap accessible. •
