You'll become a trusted clinical resource for patients, families, hospitals, senior living communities, providers, social workers, discharge planners, and referral partners throughout the Twin Cities Metro Area. Your ability to educate, build trust, and guide conversations will help ensure patients receive timely access to hospice care when it can provide the greatest benefit. You'll have the opportunity to make a meaningful difference while helping families feel supported during some of the most emotional healthcare decisions they will ever face. What You'll Be Doing.
Admissions & Care Transitions Meet with patients and families to discuss hospice services and eligibility. Conduct hospice consultations and clinical assessments. Facilitate smooth transitions into hospice care. Provide education regarding hospice philosophy, services, and care options. Support admissions and continuity of care efforts across the organization. Relationship Development Build and maintain strong relationships with hospitals, providers, senior living communities, case managers, and referral sources. Serve as a trusted hospice resource within the healthcare community. Participate in outreach and relationship-building initiatives. Help strengthen referral partnerships throughout the service area. Patient & Family Advocacy Guide patients and families through complex healthcare decisions. Help identify hospice-appropriate patients who may benefit from services. Provide compassionate education and support during difficult conversations. Ensure patients receive timely access to care and resources. Organizational Growth & Collaboration Partner closely with clinical and operational leadership. Support patient access initiatives and community outreach efforts. Collaborate with interdisciplinary team members to ensure seamless patient experiences. Contribute to the continued growth and success of Superior Hospice. What You'll Bring. Active Minnesota Registered Nurse (RN) License. Hospice experience preferred. Strong consideration given to nurses with backgrounds in ICU , Oncology , Palliative Care , ER , Case Management , Care Coordination , Discharge Planning , or other high-acuity clinical settings. Outstanding communication and relationship-building skills. Ability to educate patients, families, and healthcare professionals effectively. Comfortable working independently and managing a field-based schedule. Strong clinical judgment and professionalism.
