What You'll Do:
Admissions:
Complete eligibility assessments, patient and family education, medication reconciliation, consents, and start-of-care activities. Case Management (as assigned): Support symptom management, care planning, caregiver education, and ongoing coordination needs.
Visit Nurse & Crisis Support:
Provide routine, PRN, urgent, and support visits while responding to changing patient needs.
Collaborative Care:
Work closely with the interdisciplinary team to provide aligned care supporting patient and family goals.
Documentation & Compliance:
Maintain accurate EMR documentation and follow Medicare hospice regulations and agency standards.
What You'll Bring:
Active Minnesota Registered Nurse license. Hospice experience preferred or strong background in ICU, oncology, palliative care, ER, or high-acuity settings. Strong communication and patient/family education skills. Comfortable working independently in a field-based environment. Organized, adaptable, and confident navigating changing priorities. Reliable transportation and ability to travel throughout the Greater Twin Cities Metro Area. Veteran Friendly Employer We proudly support applicants from all branches of service.
