What You'll Do:

Admissions:

Complete eligibility assessments, patient and family education, medication reconciliation, consents, and start-of-care activities. Case Management (as assigned): Support symptom management, care planning, caregiver education, and ongoing coordination needs.

Visit Nurse & Crisis Support:

Provide routine, PRN, urgent, and support visits while responding to changing patient needs.

Collaborative Care:

Work closely with the interdisciplinary team to provide aligned care supporting patient and family goals.

Documentation & Compliance:

Maintain accurate EMR documentation and follow Medicare hospice regulations and agency standards.

What You'll Bring:

Active Minnesota Registered Nurse license. Hospice experience preferred or strong background in ICU, oncology, palliative care, ER, or high-acuity settings. Strong communication and patient/family education skills. Comfortable working independently in a field-based environment. Organized, adaptable, and confident navigating changing priorities.
