Job Description

Overview We are seeking a dedicated and detail-oriented Registered Nurse / MDS Coordinator to join our healthcare team. In this vital role, you will oversee the comprehensive assessment and documentation of residents' clinical needs, ensuring compliance with federal and state regulations, including Medicare and NCQA standards. Your expertise will support optimal patient care, facilitate accurate medical documentation, and coordinate interdisciplinary efforts to enhance quality outcomes. This position offers an exciting opportunity for a motivated nursing professional to make a meaningful impact in a dynamic healthcare environment. Duties Lead the Minimum Data Set (MDS) assessments process, ensuring timely and accurate completion in accordance with CMS guidelines and facility policies Collaborate with interdisciplinary teams—including physicians, therapists, social workers, and case managers—to develop individualized care plans that meet patient needs and regulatory standards Review medical records, clinical documentation, and coding (ICD-10, CPT) to ensure accuracy and completeness for reimbursement purposes Conduct utilization review and management activities to optimize patient care pathways while adhering to Medicare and managed care requirements Maintain proficiency in electronic health record (EHR) systems such as Epic, Cerner, Athenahealth, or eClinicalWorks for documentation review and data entry Facilitate discharge planning processes, including coordination of hospice care, outpatient services, or transfers to higher levels of care such as Level I or Level II trauma centers or PICUs Ensure compliance with HIPAA regulations by safeguarding patient information and maintaining confidentiality at all times Participate in clinical documentation improvement initiatives to enhance the quality of medical records and coding accuracy Support inpatient, outpatient, emergency medicine, ICU, pediatric (Pediatrics), primary care, and home care settings through specialized knowledge of physiology, anatomy, and medical terminology Qualifications Valid Registered Nurse (RN) license with current state licensure required Proven experience with MDS coordination within nursing homes or long-term care facilities preferred Strong knowledge of
ICD-10/ICD-9
coding systems, CPT coding, DRGs (Diagnosis-Related Groups), and Medicare regulations Familiarity with EMR/EHR systems such as Epic, Cerner, Athenahealth or eClinicalWorks is essential Experience in acute care settings including ICU or Level I/II trauma centers highly desirable Background in managed care environments or utilization review/management is advantageous Excellent understanding of medical records management, clinical documentation improvement practices, and healthcare compliance standards such as NCQA standards Ability to interpret complex medical information related to physiology, anatomy, pharmacology, and medical terminology Strong organizational skills with attention to detail; ability to manage multiple priorities efficiently Exceptional communication skills for effective collaboration across multidisciplinary teams Join us in delivering exceptional patient-centered care while advancing your career in a vibrant healthcare setting. $85,000.00 - $95,000.00 per year
