Position Overview:
We are seeking a highly skilled and dedicated MDS RN to join our team on a full-time, on-site basis. The MDS RN will be responsible for coordinating and overseeing the comprehensive assessment process for our residents, ensuring accuracy and compliance with regulatory standards. This role is vital in optimizing quality of care and ensuring appropriate reimbursement through accurate assessment and documentation.
Responsibilities:
Conduct and coordinate the completion of the resident assessment instrument (RAI) process, including MDS assessments, care planning, and ongoing resident reviews. Ensure timely and accurate completion of all assessments per regulatory requirements and facility policies. Collaborate with interdisciplinary team members to develop individualized care plans that reflect resident needs and preferences. Monitor and evaluate the effectiveness of care plans, making adjustments as needed to promote optimal resident outcomes. Maintain thorough and accurate documentation to support care plans, assessments, and reimbursement processes. Provide education and support to nursing staff related to the MDS process and regulatory guidelines. Participate in Quality Assurance and Performance Improvement (QAPI) initiatives to enhance resident care and satisfaction. Maintain knowledge of current federal and state regulations related to MDS and RAI processes.
Qualifications:
Active RN license in the state of Minnesota. Minimum of 2 years of MDS experience in a skilled nursing or long-term care setting. Knowledge of
MDS 3.0, RAI
processes, and Medicare/Medicaid reimbursement. Strong organizational and time management skills. Excellent communication and interpersonal skills. Ability to work effectively within an interdisciplinary team. Proficiency with computer systems and electronic medical records (EMR)
