Mileage Reimbursement:

Yes (between visits, from first patient to last patient)

Primary Responsibilities:

• Provide direct skilled nursing care as a Field RN in home health settings
• Manage and coordinate patient care plans in collaboration with clinical teams
• Perform patient assessments, documentation, and care interventions
• Support medication management and patient education as needed
• Ensure accurate and timely OASIS and clinical documentationpliance (if applicable)
• Communicate effectively with physicians, patients, and interdisciplinary staff
• Follow all infection control, privacy, and safety protocols
• Travel throughout assigned service areas including Moline, IL and surroundingmunities
• Participate in on-call rotation as scheduled
• Perform additional duties as assigned

Qualifications:

• Active RN license required
• Experience in home health (HH) required
• Experience with HCHB required
• Strong clinical assessment and patient care coordination skills
• Ability to work independently in a field-based role
