What You'll Be Doing As a Floating RN Case Manager, you'll provide skilled nursing care while helping ensure continuity across multiple territories and care teams. You'll perform assessments, coordinate services, educate patients and families, and deliver the high-quality care that has made Legacy Home Care and Home Health Care, Inc. a trusted name throughout Minnesota. Responsibilities Provide skilled nursing visits and comprehensive patient assessments. Develop, implement, and update individualized plans of care. Educate patients and caregivers regarding medications, safety, and disease management. Support multiple territories based on patient and staffing needs. Assist with admissions, routine visits, and case management coverage. Collaborate with therapists, social workers, physicians, and interdisciplinary team members. Complete accurate documentation, including OASIS when required. Participate in rotating on-call responsibilities. Qualifications Active Minnesota Registered Nurse License Required Strong clinical judgment and care coordination abilities. Ability to work independently in a field-based environment. Excellent communication and relationship-building skills. Home health experience preferred but not required. Valid driver's license and reliable transportation. Why Home Health?
