CHRs serve as a bridge between the Tribal community and healthcare systems, helping individuals and families access resources, manage chronic conditions, and improve overall health outcomes. Essential Duties, functions, & Responsibilities Provide one-on-one education to individuals on health-related topics through assigned home visits and walk-in visits. Will deliver culturally appropriate health education on wellness, chronic disease management, and prevention. Assist clients in bathing, blood pressure monitoring, blood glucose monitoring, and other duties as assigned by Community Health Nurse or Medical Provider. Coordinate and develop annual, quarterly, and monthly reports for Bois Forte Band of Chippewa Health and Human Services Department. Attend all training, workshops, continuing education, and meetings related to the position. Maintain current, accurate and secure files ensuring they comply with HHS and HIPAA regulations. Provide home visits, outreach, and follow-up care to community members in homes, hospitals, nursing homes, and other facilities. Organize and maintain a current file on all client referrals with pertinent data and social service resources available to the community; maintains referrals and appointments for clients to ensure that appointments have been met. Serve as a liaison between the community and health/social service resources. Collaborate with healthcare providers, Tribal programs, and community partners. Serve as an advocate for patients in navigating healthcare systems. Transport clients to health resources when clients have no other means of transportation in accordance with Bois Forte Human Services Transportation Policy. Maintain arrangements and assist with special clinics during the year, such as immunization, audiology, optometry, and other community clinics as needed. Conduct basic health screenings ( e.g. blood pressure and/blood glucose). Conduct home safety assessments for potential hazards and make appropriate contacts to remove hazards. Maintain and organize the Bois Forte Needs Program. Refers community members to the program as needed. Perform clerical duties, including typing and word processing for the Community Health Department Staff. As an employee of the Bois Forte Reservation, will follow the Bois Forte Procurement Policy and any other applicable procurement requirements when obtaining goods and/or services for the Bois Forte Band of Chippewa. Perform other duties as assigned.
MINIMUM MANDATORY QUALIFICATIONS
Experience:
None.
Education:
High School Diploma/Or Equivalent.
License/Certification:
Become a Certified Community Health Representative within 90 days. or
- Become a Certified Nursing Assistant within one year.
- Must possess a valid Minnesota driver's license, be insurable under the Band's RTC Automobile Policy, and be eligible to drive under any other motor vehicle use policies applicable to the position.

- Knowledge of Microsoft Office Suite (Word, Excel, etc.), internet software and appropriate storage of electronic files.

PREFERRED QUALIFICATIONS
- Be certified or willing to become certified as a Home Health Care Assistant or Nursing Assistant.
