What You'll Be Doing Lead Patient Care Perform comprehensive assessments and develop individualized care plans Manage ongoing visits and patient education Support patients and families throughout the recovery process Coordinate care across the healthcare continuum Deliver exceptional care that promotes independence and quality of life Use Your Clinical Expertise Manage chronic conditions and complex medical needs Perform OASIS assessments and clinical documentation Collaborate with physicians and interdisciplinary teams Identify changes in condition and intervene proactively Help reduce avoidable hospitalizations and emergency room visits Build Relationships That Matter Become a trusted resource for patients and families Provide encouragement, education, and support See the direct impact your care has over time Help patients remain safe and independent in their homes Qualifications Required Active Minnesota RN license Minimum of three years of RN experience Strong assessment and communication skills Reliable transportation and valid driver's license Passion for independent, relationship-based nursing Preferred Home health experience OASIS experience Previous case management experience Great Nurses Leave Lasting Impressions Patients may forget the details of a visit.
