What You'll Be Doing Provide skilled hospice nursing care during evening and overnight hours. Deliver pain and symptom management through a comfort-focused approach. Assess changing patient conditions and provide appropriate interventions. Support patients and families through education, communication, and compassionate care. Provide care in private homes, assisted living communities, memory care settings, and skilled nursing facilities. Coordinate closely with physicians, social workers, chaplains, hospice aides, and fellow clinicians. Complete timely documentation supporting continuity and quality outcomes. Help patients remain comfortable while honoring their goals of care. Who This Role Is Built For Active Minnesota Registered Nurse (RN) License Hospice experience preferred but not required. Strong consideration given to nurses from

ICU , ER

, Oncology , Palliative Care , Critical Care , Med Surg , Telemetry , and hospital-based nursing backgrounds. Nurses interested in transitioning into hospice are encouraged to apply.
