We are hiring a Hospice Clinical Liaison (RN) to serve as a key connector between referral sources, patients, and our clinical team , driving both admissions and patient experience across our expanding service area. Why This Role Is Different High-impact position combining clinical expertise with relationship-driven work. No fixed caseload unless needed, focus on admissions, referrals, and field support. Direct collaboration with intake and leadership influencing real-time decisions. Strong earning potential for experienced hospice or high-acuity nurses. Autonomy in the field with full clinical and operational support. Opportunity to step beyond bedside nursing into a broader, strategic role. What You'll Do Review referral pipelines, intake updates, and reports to identify hospice-appropriate patients. Conduct outreach and build relationships with hospitals, SNFs, and senior living communities . Partner with intake to prioritize referrals and coordinate timely admissions and evaluations . Complete hospice admissions including eligibility, education, consents, and start of care. Provide visit nurse support when census is high or case managers are unavailable. Communicate with discharge planners, social workers, and providers on hospice eligibility. Support census growth through consistent follow-up and strong referral relationships . Maintain accurate EMR documentation and ensure compliance with hospice regulations. Step in across the clinical team to ensure seamless patient care delivery . What We're Looking For Active Minnesota Registered Nurse license. Hospice experience preferred or strong background in ICU, oncology, palliative care, or high-acuity settings . Experience with hospice admissions, care transitions, or liaison work strongly preferred. Comfortable working independently in a field-based environment . Strong communication, relationship-building, and clinical judgment skills. Highly organized, adaptable, and able to manage changing priorities.
