Job Description

Company Overview:
Spring Valley Living is dedicated to providing exceptional housing and care services to our residents, tenants, and outpatients. With a committed team of around 100 nursing staff and professionals, we focus on delivering high-quality experiences rooted in dignity and respect for those we serve.
Summary:
We are seeking an RN Care Coordinator for our skilled nursing facility. In this role, you will coordinate the delivery of services to managed care and Medicare residents, provide direct nursing care to residents, and supervise the day-to-day nursing activities performed by nursing assistants. Such supervision must be in accordance with current federal, state and local standards, guidelines, and regulations that govern our facility, and as may be required by the Director of Nursing to ensure that the highest degree of quality care is always maintained.
Opening:
Full-Time Day, M-F, includes on-call responsibilities, and may be subject to an occasional weekend as needed.
Duties may include:
Assist in developing and implementation of nursing service objectives, policies and procedures Participate in admit, transfer, and discharge of residents Participate in the orientation of new residents/family members to the facility Educate/teach disease process to LTC or STC residents MDS set up and completion for assigned unit Assist in monitoring infection control practices and wound care management Participate in IDT meetings and care planning process, regularly review care plans and assessments Complete accident/incident reports as necessary Develop work assignments and/or assist in completing and performing such assignment. Complete routine monitoring of cares to ensure staff has adequate job skills/knowledge. Facilitate competency testing for licensed and unlicensed staff Complete any other tasks as deemed necessary by your supervisor
Qualifications:
Active RN license in the state of Minnesota Previous nursing experience in Long-Term Care preferred Prior MDS experience is a plus Spring Valley Living is a proud Equal Opportunity Employer!
