As a Hospice RN Resource Nurse , you'll help support admissions, patient visits, case management coverage, and clinical needs throughout the Twin Cities Metro Area. A Position That Keeps You Learning Many hospice organizations place nurses into a single lane. Admissions. Case Management. Visit Nursing. This role is different. As a Hospice RN Resource Nurse , you'll become a trusted clinical resource capable of stepping into multiple situations where support is needed. Your ability to adapt and provide exceptional care helps create continuity for patients while strengthening the entire clinical team. For nurses who enjoy learning, solving problems, and making a difference wherever they are needed most, this role offers a level of variety that is difficult to find elsewhere. What You'll Be DoingAdmissions Support Complete hospice eligibility assessments and admission visits. Provide patient and family education regarding hospice services. Obtain consents and admission documentation. Perform medication reconciliation. Facilitate smooth and supportive transitions onto hospice care. Case Management Support Assist with care planning and ongoing patient management. Support symptom management interventions. Educate caregivers and family members. Coordinate care with patients, families, and interdisciplinary team members. Patient Visits & Clinical Coverage Conduct routine, PRN, urgent, and support visits. Respond to changing patient conditions and clinical needs. Provide coverage support throughout the service area. Help ensure continuity of care during staffing transitions and scheduling needs. Interdisciplinary Team Collaboration Partner closely with nurses, social workers, chaplains, hospice aides, providers, and leadership. Participate in care planning discussions. Support patient-centered goals of care through collaborative decision-making. Documentation & Compliance Maintain accurate and timely EMR documentation. Follow Medicare hospice regulations and agency standards. Ensure compliance with clinical documentation requirements. What You'll Bring Active Minnesota Registered Nurse (RN) License. Hospice experience preferred. Strong consideration given to nurses from
ICU , ER
, Oncology , Palliative Care , Critical Care , or other high-acuity clinical settings. Excellent communication and patient/family education skills. Ability to work independently in a field-based environment. Strong clinical judgment and critical thinking abilities. Organized, adaptable, and comfortable managing changing priorities. Reliable transportation and ability to travel throughout the Greater Twin Cities Metro Area.
