Assisted Living Triage On-Call RN

Overview Join our dedicated healthcare team as an Assisted Living Triage On-Call Registered Nurse (RN), where your expertise will directly impact the quality of life for residents in assisted living communities. In this vital role, you will provide triage assistance for after hours needs. Responsibilities Conduct remote basic triage assessments for residents experiencing health concerns, utilizing medical terminology and physiology knowledge to determine urgency and appropriate intervention. Provide real-time guidance to on-site staff regarding symptom management. Collaborate with multidisciplinary teams including physicians, therapists, and care coordinators to develop and implement individualized care plans aligned with NCQA standards and HIPAA regulations. Document all patient interactions accurately within electronic health record (EHR) systems such as Epic or Cerner, ensuring compliance with medical documentation protocols. Respond promptly to urgent situations such as acute pain management episodes or infectious disease concerns within the assisted living environment. Requirements Valid RN license with current state registration; prior experience working in assisted living, nursing home, or long-term care settings is highly preferred. Proficiency in EMR/EHR systems such as Epic or Cerner; experience with medical imaging, laboratory experience, and ICD-10 coding is desirable. Strong knowledge of medical terminology, anatomy, physiology, and diagnostic evaluation techniques including venipuncture and catheter care. Ability to perform patient assessments involving vital signs, physical examinations, and intake procedures confidently while adhering to aseptic technique standards.
