Job Description

About the Role We are seeking a compassionate and detail-oriented Facility Nurse to join our team in a Minnesota 245D-licensed program. This role is essential in supporting individuals with disabilities by ensuring high-quality healthcare services, regulatory compliance, and person-centered care. Key Responsibilities Provide and oversee nursing services in accordance with Minnesota 245D Home and Community-Based Services (HCBS) standards Complete health assessments and develop individualized health service plans Administer medications and treatments as prescribed Delegate nursing tasks to unlicensed staff Train and supervise direct support staff on medication administration and health-related procedures Monitor and document health status, incidents, and progress notes Coordinate care with physicians, case managers, and interdisciplinary teams Ensure compliance with DHS licensing requirements and maintain accurate documentation Participate in audits, quality assurance, and program improvement initiatives Qualifications Current Minnesota RN or LPN license in good standing Experience working with individuals with developmental disabilities or in a 245D setting preferred Knowledge of
MN DHS 245D
regulations and HCBS requirements preferred Strong assessment, documentation, and communication skills Ability to delegate, train, and supervise staff effectively CPR/First Aid certification (or willingness to obtain)
